- $5.00 plus state/local sales tax monthly service charge if the account falls below $500.00 on any day during the statement cycle.

- $100.00 minimum to open account.
- $5.00 plus local/state sales tax monthly service charge if the account falls below $2500.00 on any day during the statement cycle.

- $5.00 plus state/local sales tax monthly service charge if account falls below $15,000.00 on any day during the statement cycle.

- $5.00 plus state/local sales tax service charge if the average daily balance falls below $15,000.00 on any day during the statement cycle.

Health Savings Account
